The Integrated Community Stroke Service (ICSS) at Home is for people with a diagnosis of stroke or non-traumatic subarachnoid haemorrhage in Bristol, North Somerset and South Gloucestershire.
Who is this service for?
What we do
ICSS at Home supports you to get home from hospital as soon as possible after a stroke and provides rehabilitation, advice and support to help you regain your independence and adapt to life after stroke.
We work with the hospital team to plan what you will need at home and will arrange any equipment, such as handrails or mobility aids. We aim to see most people on the day you get home from hospital so that we can check that you have everything you need and then work together to make a plan for your rehabilitation.
We will continue to see you until: you do not have any more therapy goals; those goals can be worked towards without the team’s support; or goals are not being achieved.
Before we stop our visits we will make sure that you have the strategies you need to set your own longer term goals and work towards them without our support.
We will link you to long-term Life After Stroke services provided by the Stroke Association and Bristol After Stroke and our team of stroke specialist nurses who can provide support and advice. You will be offered a nursing review and six month review.

ICSS at Home takes referrals for people with goals related to their stroke symptoms in Bristol, North Somerset or South Gloucestershire.
If you wish to self-refer then contact the ICSS team and one of the team will discuss your needs and arrange a visit, give advice or signpost you to a more appropriate service.
