Diabetes and Nutrition Services

Our Diabetes and Nutrition Service (DANS) provides education, information and support on diabetes and nutrition to people registered with a GP  in Bristol, North Somerset and South Gloucestershire and their carers. The team also support healthcare professionals in the area.

The diabetes team consist of diabetes specialist nurses and dietitians who work in the community to support people living with complex diabetes needs. We also provide diabetes structured education for people living with diabetes.

For healthcare professionals who support people living with diabetes, we offer the following services:

• Education courses – Diabetes Foundation, Insulin initiation in Type 2 diabetes.
• Telephone and email advice and guidance line.
• Joint working in practices to support improving the skill of clinicians in primary care. 
• Virtual clinics in GP practices. 
• Skill building or clinical update sessions for GP practices. 
• Provision and supply of Type 2 diabetes information packs. 

The Diabetes and Nutrition Service accepts referrals from any healthcare professional for Diabetes education courses, self-referral for the Diabetes education courses can be considered. Please telephone the number on the right to discuss your self-referral.

Referrals to the services other than structured education need to be made by your GP or Lead Clinician.
