Elton Rehabilitation Unit

Elton Rehabilitation Unit is an inpatient rehabilitation service situated on the first floor of North Somerset Community Hospital. This service is for adults over the age of 18.

People in our care receive rehabilitation and recovery at Elton Unit when they can’t be safely discharged home following a hospital stay. This may include support to relearn activities of daily living such as cooking meals and washing. They will be supported by a multidisciplinary team of clinicians with different specialties.

The Unit provides care which cannot be given in someone’s usual residence. However as soon care can continue at home, we will ensure they are supported to do so.

A formal referral from a hospital professional is required to discharge from hospital into a community bed at the Unit.

A referral will be completed at the hospital and will be managed by the Transfer of Care Hub for a ‘step down’ or ‘Pathway 2’ bed at the Unit.

Visiting times on the Unit are 11am to 7pm, but we ask that you avoid visiting at mealtimes, as our units participate in the national protected mealtimes initiative. If visiting at meal times, you will be asked to leave so that the people in our care can eat without being interrupted.

Visiting for people under the age of 18 will be at the discretion of the Unit Lead or the nurse in charge. These visits, if agreed, will need to be prebooked with the team.

We will work with you and your family so you can return home as soon as you assessed as able to do so. All our planning will be centred around you and your family or carers. We will discuss this with all involved prior to your discharge date, and will include all appropriate community services to make sure you are well supported once you leave us.

You will be given information about this during your stay and we encourage you to ask questions if you need any more information to help you to make the best choices around your discharge.
