The Frailty and Falls Service helps people who are living with frailty or who may be at risk of falling. Our team works with you to understand your health needs and provide advice to help you remain safe, active and independent.

What Is the Frailty and Falls Service?

We are a team of healthcare professionals with specialist knowledge in frailty and falls prevention.

We work with people whose health, mobility or balance may place them at greater risk of falling, helping them manage safely in the community and maintain their quality of life.

We also provide specialist advice and recommendations to GPs and other healthcare professionals involved in your care.

Why Have I Been Referred?

You may have been referred to the service because there are concerns about:

  • Falls or near misses
  • Balance or mobility difficulties
  • Frailty and general health
  • Factors that may increase your risk of falling

Our assessment helps identify any underlying issues that may be affecting your health and wellbeing.

Who Will I See?

You may be seen by one or several members of the Frailty and Falls team.

In most cases, assessments take place on the same day, although occasionally a second appointment may be required.

Family members and carers are welcome to attend appointments with you.

How Long Will My Appointment Last?

Appointments usually last between one and one and a half hours.

Before your appointment, please have available:

  • Your walking aid (if you use one)
  • A list of your current medications
  • Details of any over-the-counter medicines
  • Any herbal remedies you take

What Happens During the Assessment?

During your appointment, we will talk to you about:

  • Your medical history
  • Your day-to-day activities
  • Your home environment
  • Your memory and wellbeing

We may also:

  • Check your mobility and walking
  • Assess your balance
  • Carry out simple observations
  • Discuss any concerns you may have

This information helps us build a complete picture of your needs and identify ways to support you.

What Happens Next?

After your assessment, we will send a letter to both you and your GP outlining our recommendations.

You can then discuss any next steps with your GP or healthcare team.

Depending on your needs, we may also arrange:

  • Follow-up appointments
  • Referrals to other services
  • Additional support or advice

Take Action to Prevent a Fall

There are simple steps you can take to reduce your risk of falling:

  • Keep active
  • Eat well and drink plenty of fluids
  • Have regular eye tests
  • Ask your pharmacist for a medication review
  • Wear well-fitting shoes
  • Look after your feet
  • Remove unnecessary clutter
  • Keep a light near your bed
  • Secure loose wires and cables
  • Ensure rugs and carpets are safe
  • Limit alcohol intake
  • Use your walking aid if provided
  • Wear your pendant or wrist alarm if you have one
