What Is the Community Parkinson’s Service?

The Community Parkinson’s Service provides specialist assessment, advice and ongoing support for adults living with Parkinson’s disease. The service aims to help people manage their symptoms, maintain independence and improve their quality of life through personalised care and education.

The team works closely with patients, families, carers and other healthcare professionals to ensure the right support is available throughout every stage of Parkinson’s.


Who Can Access the Service?

The service is available to adults who:

  • Have been diagnosed with any type of Parkinson’s disease.
  • Are registered with a GP in Bristol, North Somerset or South Gloucestershire.
  • Have been referred by a consultant, GP or another healthcare professional.

What Does the Service Provide?

The service is delivered by experienced Parkinson’s Nurse Specialists (PNS) and Parkinson’s Specialist Practitioners (PSP) who provide expert advice, education and ongoing support.

Support may include:

  • Specialist assessment at home or in clinic.
  • Telephone and email advice.
  • Monitoring and reviewing Parkinson’s medication.
  • Education about Parkinson’s and symptom management.
  • Support for family members and carers.
  • Working alongside GPs and other healthcare professionals.

The aim is to help people feel confident managing their condition while providing access to specialist expertise when needed.

If additional support is required, the Parkinson’s team can help you access other healthcare professionals and local support services.

Referrals may include:

  • Physiotherapy.
  • Occupational Therapy.
  • Speech and Language Therapy.
  • Exercise programmes.
  • Local support groups.
  • Monthly Parkinson’s drop-in sessions.

How Do I Access the Service?

Access usually begins following a referral from your:

  • Consultant.
  • GP.
  • Hospital team.
  • Another healthcare professional.

Once referred, you can continue to access the service whenever ongoing support or advice is needed using the service contact details.
