What is the Dementia Advisor Service?

A Dementia Advisor provides support and advice to understand dementia and to promote an individual’s quality of life in their home and community.

The Dementia Advisor Service supports adults of all ages with symptoms of mild to moderate dementia who have already received a diagnosis of dementia. The support is also provided to carers.

The service works in partnership with GP surgeries for anyone registered with
a South Gloucestershire GP. Currently the service is provided by one Dementia
Advisor in each of the following areas: Thornbury, Patchway and Yate.

Our Service Offers:

  • Help for you and your family to understand the diagnosis of dementia
  • Emotional support for you and your carer to cope with the ongoing challenges of living with dementia
    Discussing and explaining ideas and tips to enable you to live well in your own home, including how to prevent falls and accidents
  • Informing you about local and national support services, including local memory cafes and activity groups
  • Support and advice for your family and carers who are caring for the person with dementia
  • Future planning for your health and wellbeing wishes
  • We work in partnership with your GP surgery and other health and social care professionals

Your Information

With your consent information may also be shared with other professionals
involved in your care.

Your information may be used anonymously by the National Health Service
(NHS) to provide data to develop future services.

Somerset and surrounding areas.
