When a patient safety incident occurs, we are committed to understanding what happened, supporting those affected and learning from the experience. By identifying opportunities for improvement, we can continue to make our services safer for everyone.

What is the Patient Safety Incident Response Framework (PSIRF)?

The Patient Safety Incident Response Framework (PSIRF) is the NHS’s national approach to responding to patient safety incidents. It encourages healthcare organisations to focus on learning and continuous improvement, rather than carrying out the same type of investigation for every incident.

This flexible approach helps us identify the most appropriate response based on the circumstances of each incident and where the greatest opportunities for learning exist.


How We Respond to Patient Safety Incidents

Every patient safety incident reported to Sirona Care & Health is carefully reviewed.

Depending on the nature of the incident, our response may include:

  • Reviewing clinical records
  • Speaking with the healthcare professionals involved
  • Understanding how systems and processes contributed
  • Identifying improvements that can make care safer
  • Sharing learning across our organisation

In some situations, we will carry out a Learning Response, which provides a structured way of understanding what happened and identifying improvements.


Involving Patients, Families and Carers

If you or someone you care for has been affected by a patient safety incident, you may be invited to share your experience.

A Family Liaison Officer may contact you to explain the process, answer any questions and discuss how you would like to be involved.

Your experience helps us understand:

  • What happened from your perspective
  • How the incident affected you
  • What improvements could make care safer for others

Taking part is entirely your choice, and we will always listen with compassion and respect.

Sirona Care & Health values the experiences of patients, families and carers.

Patient Safety Partners work alongside healthcare professionals to help improve services, contribute to patient safety initiatives and ensure that the patient voice is represented when decisions are made.
