Specialist Community Neurology Service
Supporting adults with neurological conditions in the community
The Specialist Community Neurology Service provides NHS-funded therapy and rehabilitation for adults living with neurological conditions.
Our aim is to help you maximise your independence, improve your quality of life, and support you in achieving the goals that matter most to you.
The service is available for people who are registered with a GP in Bristol, North Somerset or South Gloucestershire (BNSSG).
Who Can Use This Service?
You may be able to access the service if you:
- Are aged 18 or over.
- Have a neurological condition affecting your daily life.
- Are registered with a GP in Bristol, North Somerset or South Gloucestershire.
Your therapy will be tailored to your individual needs and circumstances.
Meet Our Team
Our multidisciplinary team brings together professionals with a range of expertise to provide joined-up neurological care.
The team includes:
- Occupational Therapists
- Physiotherapists
- Speech and Language Therapists
- Therapy Assistants
- Administrators
- Multiple Sclerosis Specialist Nurses (North Somerset only)
By working together, we can provide coordinated care that focuses on your personal goals.
Where Will We See You?
We aim to provide appointments in the place that best meets your needs.
This may include:
- Your home
- Your workplace
- Your college or place of education
- Telephone appointments
- Video consultations where appropriate
Your therapist will discuss the most suitable option with you.

Assessment
We assess how your neurological condition is affecting your daily life and identify areas where support may help.
Goal-Based Therapy
Together, you and your therapist will agree realistic goals and develop a personalised therapy plan to help you achieve them.
Advice and Support
We provide information and practical advice for you, your family and anyone involved in your care.
Working Together
We work closely with a range of professionals and organisations, including:
- GPs
- Hospital teams
- Social care services
- Specialist nurses
- Voluntary organisations
This joined-up approach helps ensure you receive coordinated care.
Electronic Patient Records
To help our team communicate effectively and provide safe care, we use an electronic patient record system.
This allows members of your care team to access relevant information and work together more efficiently.

How to Access the Service
If you have used the service before
If you have been seen by the Specialist Community Neurology Service within the last two years, you may be able to contact us directly by telephone or email to discuss referring yourself back into the service.
If you are new to the service
If you have not previously been under our care, a healthcare professional involved in your treatment can refer you.
Waiting for an Appointment
Following referral, you will be placed on our waiting list.
We will contact you as soon as an appointment becomes available.
If your condition changes while you are waiting, or you would like an update on waiting times, please contact the team.
If You Need to Cancel an Appointment
We understand that appointments sometimes need to be rearranged.
If you need to cancel, please let us know as soon as possible.
Frequent cancellations or missed appointments may mean your therapist needs to discuss whether this is the right time for the service to remain involved in your care.
This helps ensure appointments are available for everyone who needs them.
Please note that our telephone line operates as an answerphone service, so messages may not be picked up until the next working day.
