
Husband and wife team Jon and Hollie Moore are helping to keep people out of hospital this winter as part of a specialist team run by Sirona care & health. Jon and Hollie have both worked in the NHS for over a decade;
Jon is a Paramedic who previously worked in pre-hospital emergency care with the ambulance service while Hollie, who first joined Sirona as a District Nurse Team Leader, has worked in specialities such as renal care and the Emergency Department at Southmead Hospital in Bristol where they met.
The duo now both work for Sirona care & health, a not-for-profit social enterprise providing community health care across South Gloucestershire, part of Bristol and Bath and North East Somerset.
Jon and Hollie are part of an 11 strong team of Emergency Care Practitioners (ECPs), made up of Advanced Paramedics and Nurses, who visit people at home throughout South Gloucestershire.
Working closely with GP surgeries, the ECP service provides a same-day visiting service for people who are seriously unwell, have complex conditions and face being admitted to hospital if they do not improve.
Jon said: “We are able to use our advanced training and specialist knowledge to provide thorough clinical and whole-person assessments. We use a ‘here and now’ approach to providing treatments and completing tests which people would otherwise need to see multiple professionals.”
As well as working with GPs, the ECPs are able to call upon other Sirona or social care services to help them support people at home avoiding unnecessary admissions to hospital.
Hollie said “It is a unique service which straddles the traditional boundaries of care provided by GPs, in the community and in hospital. We provide holistic care which enables people to remain home whenever possible.”
Jon explains “We see people whose conditions are urgent, complex in nature or at risk of rapid deterioration; all of our patients need to and are seen the same day. As a team we are passionate about providing a service which enables us to spend whatever amount of time is needed with each individual. We can ensure every person’s health and social needs are met to keep them safe despite being outside of a hospital.”
