An exciting opportunity has arisen for a highly motivated Clinician – Registered Nurse / Paramedic – to join our forward thinking Single Point of Access Team (SPA) in North Somerset. This would be a great opportunity to consolidate your knowledge and develop as a B6 triage clinician by enhancing your management and leadership skills alongside learning the art of telephone triage.
The role is stimulating and often fast paced and you will be required to triage telephone and electronic referrals identifying the level of need and ensuring a safe and appropriate decision that reflects the patient’s clinical need. You should be enthusiastic, positive and motivated, possess a high level of both clinical and communication skills, be able to use your own initiative and work well as part of a multidisciplinary team.
You will be required to have an up to date broad knowledge of clinical practice and relevant experience to enable you to accurately triage referrals received over the telephone and via our clinical system. Basic IT skills are required although additional training will be given once in post. Knowledge in the working practices of SPA would also be beneficial.
Based in Castlewood in Clevedon the Single Point of Access (SPA) is the first point of contact for, and entry into adult community services for a range of health and social care professionals, Service Users, their relatives and carers, and other stakeholders within Sirona and the wider health community. We receive referrals for acute, chronic and planned care within the community setting. The focus centers on delivering a high standard of care to patients; facilitating discharge from hospital, and helping to prevent avoidable admission into hospital. We pride ourselves in safe, effective and efficient triage enabling patients to receive the right care, at the right time, and in the right place with onward referrals and sign posting into the appropriate Sirona community services.
SPA is operational 7 days a week 08.00 – 20.00 hours and you will be required to work varied rostered shifts, including weekends and bank holidays within these hours.
