Our Adult Learning Disability Health Service has launched their new Learning Disability Health Passport.
The passport is for anyone with a learning disability or learning difficulty. It is an important document which tells health and social care staff about a person’s needs such as, medication, personal details and how they like to communicate. It should be taken to healthcare appointments to help a person get the best care.
The new passport will be used in all health and social care settings, in line with new NHSE guidance. It replaces previous hospital passports used in local hospitals.
The Health Passport is a live document owned by the person and read by health care professionals. The Disability Health Passport and an easy-read version are available on the Sirona website on the team’s page.
Already complete hospital passports will be used while we phase onto the new Health Passport.
