CCHP services transfer: what this means for you

A message from your new provider, Sirona care & health

We’re Sirona care & health and we are delighted to be delivering Community Children’s Health Partnership Services (CCHP) in Bristol directly from 1 February 2020.

We are working with Bristol, North Somerset and South Gloucestershire Clinical Commissioning Group to transform the way children, young people and their parents and carers receive care and support and we are about helping and supporting people to meet their individual goals, not fitting people to services – we know one size does not fit all.

We are a third sector – not-for-profit – organisation funded by the NHS and we have been providing CCHP services in partnership with others since 2016.

We are committed to:

  • listening, involving and learning from you, your families/carers and your communities
  • transforming services to make a positive difference by building on what is already good
  • being inclusive and ensuring equal access to services.

Protecting your personal information

So we can continue providing you with great services, Bristol Community Health will transfer your records securely to us. This information will only be used to provide you with services and we will only share it with people involved in your care.

A message from your existing provider, Bristol Community Health

From 1 February 2020, Sirona care & health will be providing the community health and care services currently delivered by Bristol Community Health. It’s been an honour for Bristol Community Health to support those within our communities over the last 8 years.

What does this mean for you?

We’re working closely with Sirona to transfer the services without any interruption to you. In most cases, you won’t notice any changes as the people who provide your care and support will stay the same.

Your care and appointments will continue as planned from 1 February 2020 and we will securely transfer your current Bristol Community Health records to Sirona to support this.

Do you need to do anything?

If your treatment will be complete when our service finishes on 31 January 2020 then we will archive your Bristol Community Health records securely.
