Children in Care and Adoption Team
Who is the service for?
The service is for children and young people aged 0-18 years that are looked after. We also support those involved in their care such as carers, social workers and education settings.
We also provide support to those who have left care.
Children and young people are referred into our service through the local authorities or Children in Care health teams in other areas.
What we do
We are a dedicated health team who support children and young people who are in care or in the process of being adopted.
Our team promotes the health and wellbeing of children and young people by working closely with those involved in their care including:
- Parents and other family members
- Foster carers
- Social workers
- Leaving care personal advisors
- Teachers / virtual school
- School nurses and health visitors
- GPs and paediatricians
