Anyone wishing to make a referral into the following Community Children’s Services:
- Community Paediatrics
- Neurodevelopmental Pathways (ADHD and Autism)
- Speech and Language Therapy
- Occupational Therapy and Physiotherapy
- Eating, Drinking and Swallowing Difficulties
- Youth Justice
- Stammering
- The Children’s Bladder and Bowel Service
- Early Years Support Practitioner (SEND) Service

The Single Point of Entry Team is responsible for ensuring all referrals are processed efficiently, accurately, and securely resulting in better outcomes for children and families.

Getting help
To help us process your referral as quickly as possible, please submit the completed SPE referral form together with any mandatory supporting documents listed.

Please ensure all sections are completed and any mandatory documents are included otherwise the referral may be returned to the sender.

If your child is experiencing any continence difficulties, please discuss with your child’s GP, Health Visitor, School Nurse or any other health professional working with your child.
Early Years Support Practitioner (SEND) Service
