  • Get health checks, and access to other community services that can help you manage your health.

Who are Community Appointment Days for?

Community Appointment Days are for people who:

  • have been invited by one of our services.
  • may benefit from care that is coordinated across different teams or professionals.
  • would welcome a personalised approach.

What will happen on the day?

Arrival and welcome

  • you may be asked to complete some questionnaires prior and after your appointment.
  • you will have a Health Passport for you and your health professional to complete during your appointment.
  • the space is open plan with other staff and people in the same room.

Your appointments

  • Please wear or bring appropriate clothing (eg a knee problem you may wish to bring shorts or a vest for a shoulder problem).
  • You will have a 1:1 appointment with a health professional focusing on  listening and understanding what matters to you.

How long will I be there?

  • The length of your appointment will vary depending on what you need, but people typically people spend at least an hour at the event.
  • The team will aim to make the day as smooth and comfortable as possible and keep you informed.

Do I need to prepare anything?

  • Bring your appoint letter or message.
  • Bring a list of medication.
  • Consider what matters to you in your health before the day.
  • Think of any questions or concerns you would to discuss.
  • Please bring reading glasses, hearing aids if needed and wear appropriate clothing depending on what your problem is.

Accessibility and support

We want everyone to feel comfortable and supported.

  • Mobility or access support
  • An interpreter
  • Extra time or a quieter space
  • Information in a different format
