This stroke specialist nursing team is part of the Integrated Community Stroke Service (ICSS) at Home. ICSS at Home is for people with a new diagnosis of stroke in Bristol, North Somerset and South Gloucestershire.
Who is this service for?
What we do
For people who are discharged from hospital after a stroke without any rehabilitation needs, our stroke specialist nurses will contact you at home about six weeks after you leave hospital.
We’ll discuss how your stroke is affecting you and see if we can help in any way. We’ll talk about things like continence, sexual activity, diet and lifestyle
We provide information, support and advice to you and your partner, relatives and carers, and will discuss any concerns with you. The nurse may also arrange to visit you at home if necessary.

How do I access this service?
ICSS at Home takes referrals for people with a new diagnosis of stroke in Bristol, North Somerset or South Gloucestershire via hospital referral by a healthcare professional. You can’t currently self-refer to the service.
