A learning disability affects the way a person understands information and how they communicate.

A person with a learning disability may have more specific problems processing certain forms of information.
Our Adult Learning Disability Health Service work with people to help them to understand and make choices about their health. Our teams are made up of different professionals who specialise in working with adults who have a learning disability. Sometimes we work directly with the person who needs support. We also work indirectly, through family or support workers. We help set goals and make plans to help meet needs.
Our learning disability health passports
Our Learning Disability Health Passport has been developed with our partners and is used to ensure services from different organisations know important medical and personal information about the people in our care.
