Our Care Home Support Team is available to support care homes across the Bristol, North Somerset and South Gloucestershire area. We support adults in care homes and care home staff across the area.
Who is this service for?
What do we do?
We are a non-urgent service made up of four area team leaders and a physiotherapist. We provide information, training, support sessions, and advice to care home staff.
We provide action plans to support with safeguarding concerns and use data to help support care home teams to improve the health and care of their residents.
How do I access this service?
To be seen by the team, you must be referred by a care home member of staff or healthcare professional.
I’m a clinician – how do I make a referral to this service?
Care home managers, staff and other professionals can refer via the team inbox: sirona.carehomes@nhs.net, which is monitored between Monday and Friday in office hours (8-4). It the matter is urgent then please contact our Single Point of Access.
