End of Life and Palliative Care
This service is for people in the last year of life. We aim to help those with advanced, progressive and terminal conditions to live as well as possible and die with dignity.
The people providing your care will ask you about your wishes and preferences, and take these into account as they work with you to plan your care. They will also support your family, carers or other people who are important to you.
To access this service you’ll need to be referred by a healthcare professional.
