Who is this service for?
Anyone who is rough sleeping or in temporary/emergency accommodation in Bristol, North Somerset and South Gloucestershire.
What do we do?
We provide holistic health care intervention to people who are homeless, or in emergency/temporary accommodation across Bristol, North Somerset and South Gloucestershire. The team supports patients, bridging the gap between services aiming to reduce hospital attendance and admission and promote engagement with appropriate community and primary care services.
How do I access this service?
People can be referred by health care professionals, support workers or refer themselves using our referral form.
