Who is this service for?
This service is for people who are registered with a South Gloucestershire GP, requiring short term intensive programmes of rehabilitation to either maintain independence, prevent hospital admission or facilitate discharge from hospital.
What we do
The main aims of the service are:
- To prevent unnecessary admission to hospital
- To facilitate discharge from hospital
- To support people to regain independence through a planned programme of therapy
How do I access this service?
You can be referred to the service by:
- a GP
- community nursing team
- community matron
- hospital staff if you are an inpatient
