Who is this service for?
This Service is available for people living in Bristol, North Somerset and South Gloucestershire who need urgent care in their own homes.
What we do
Practitioners with extended training undertake urgent home visits at the request of GPs as part of the adult community services multidisciplinary team. They undertake physical assessments and look to either set up a treatment plan, with or without a support package, give self-care advice or if necessary admit to secondary care for acute care.

You can be referred by your GP or other health care professional.
