Who is this service for?

We are a community-based Heart Failure Service and see people aged over 18 who:

  • Have seen their GP who suspects a new diagnosis of heart failure.
  • Have previously been diagnosed with heart failure and have worsening symptoms.
  • Have recently been discharged from hospital and diagnosed with heart failure.
  • Already have a heart failure diagnosis but may need a treatment review.

What we do

We are a team of Heart Failure Specialist Clinicians, Assistant Practitioners and Administrators, who work in partnership with Consultant Cardiologists and Cardiac Sonographers to diagnose and treat heart failure, monitor symptoms and manage medications. We work closely with primary and community care professionals.

We support and empower patients to manage their heart failure and offer a wide range of community clinics throughout across Bristol, North Somerset and South Gloucestershire.

We provide telephone advice and guidance for GPs and healthcare professionals Monday to Friday (2-4pm).

How do I access this service?

Referrals can only be accepted for people who are registered with a GP in Bristol, North Somerset or South Gloucestershire.

Referral to this service can only be made through your GP or a healthcare professional . Self-referrals are not accepted.

Please make an appointment to discuss your concerns with your GP in the first instance.
