NHS@Home

NHS@Home is our name for virtual wards in the Bristol, North Somerset and South Gloucestershire area. It provides an alternative option for people who become unwell and would normally be spending time in hospital, to receive hospital-level care, monitoring and treatment at home, helping speed up their recovery

The service is a partnership between different NHS healthcare organisations and technology company called Doccla.

We offer healthcare in your home or normal place of residence, rather than in hospital. This means you can spend less time in hospital, or avoid being admitted altogether.

Just as in hospital, people in NHS@Home are cared for by a multidisciplinary team who can provide a range of tests and treatments. This could include blood tests, prescribing medication or administering medications through an intravenous drip.

People in our care are reviewed daily by the clinical team and may involve a home visit or take place through video technology. We also use technology like monitoring and other medical devices enabling clinical staff to have real-time information on vital signs and easily check in and monitor the person’s recovery.

We can be contacted by calling 0300 125 5000 or by messaging us on the smartphone provided. If required, a home visit by a member of the team can also be arranged.

You will be supported by the service for a short period of time, up to 14 days, but we will discuss progress with you regularly. Following the completion of your care, you will be discharged from the service. If necessary, you may return to hospital for further treatment. If you become unwell whilst with us, we will make sure you can access hospital care quickly, and in a planned way.

You can be referred to the service by any health care professional.
