Who is this service for?

This service is for people of all ages with a diagnosis of Parkinson’s. You must be registered with a Bristol, North Somerset and South Gloucestershire GP practice.

What we do

Our Parkinson’s Service can provide information, advice and support for people with Parkinson’s and for the carers, families and healthcare professionals involved in their care. Our service includes both Parkinson’s Nurse Specialists and Parkinson’s Specialist practitioners who carry out the same role.

We offer:

  • A full assessment of the needs of each person with Parkinson’s and monitor and review the effectiveness of Parkinson’s medication
  • Clinic appointments within the community setting, home visits for the housebound and also video or telephone appointments if required.
  • Telephone and e-mail support

We also:

  • Liaise with other Healthcare Professionals and agencies involved in caring for a person with Parkinson’s
  • Can be a source of local information and are able to signpost/refer to other services/agencies
  • Help to educate individuals, groups, and multidisciplinary Health and Social Care professionals to help promote a greater understanding and awareness of Parkinson’s.

How do I access this service?

Initial referral is via your Consultant, GP, or any other Healthcare Professional involved in your care.

Self-referral is encouraged once you are known to the service. Please see our Parkinson’s Referral Criteria and Referral Form to make your referral and contact the service with any questions.
