Who is this service for?

The Single Point of Access (SPA) is the number to call for those receiving healthcare services in their homes from community teams including District Nurses, Therapists and Support Workers.

We operate a multi-disciplinary team consisting of health navigators and triage clinicians, and including nurses, midwives, physiotherapists, paramedics and other specialists. We can direct you to the most appropriate adult community service for your needs.

It is also the place where clinicians can refer people into community health services no matter where they live in Bristol, North Somerset and South Gloucestershire, including GP urgent referrals that require a response within 2 hours.

Please note, we can only direct you to community services. You can see more information about other local NHS services or dial 111 or go to 111.nhs.uk at any time, if you require urgent medical help but you’re not sure where to go.

How do I access this service?

You may contact our friendly SPA team by calling 0300 125 6789 daily between 7:00am-10pm. Outside of these hours, this number will be answered by colleagues in Sirona’s Out of Hours Service who will be able to assist you.

I’m a clinician – how do I make a referral to this service?

Urgent referrals

Any new community nursing referrals that are deemed urgent (where a visit is required within 24 hours) must be made by phoning the SPA on 0300 125 6789.

Non-urgent referrals

If the visit is not urgent, you should make a referral via either email or a managed referrals on EMIS.

If emailing, please make sure you complete the non-urgent referral form fully and return to SPA as an attachment to the email address for your area:

We aim to process non-urgent referrals within 48hrs of receipt.
