A message from your new provider, Sirona care & health

We’re Sirona care & health and we are delighted to be the new provider of adult and children’s community health and care services in Bristol, North Somerset and South Gloucestershire (BNSSG) from 1 April 2020.

We are working with BNSSG Clinical Commissioning Group to transform the way you receive care and support in your home and your local community and we are about helping and supporting people to meet their individual goals, not fitting people to services – we know one size does not fit all.

We are a third sector – not-for-profit – organisation funded by the NHS and we are committed to:

  • Listening, involving and learning from you, your families/carers and your communities
  • Transforming services to make a positive difference by building on what is already good
  • Being inclusive and ensuring equal access to services.

So we can continue providing you with great services, North Somerset Community Partnership will transfer your records securely to us. A message from your existing provider, North Somerset Community Partnership

As you may be aware, on 1 April 2020 the provision of community health services in North Somerset will transfer from North Somerset Community Partnership (NSCP) to Sirona care & health (Sirona). In preparation for this, NSCP has been working closely with Sirona to ensure the delivery of your / your child’s care will be transferred seamlessly.

In line with NHS guidelines, our healthcare staff will transfer to Sirona and, to maintain the continuity and safety of your/your child’s care, we will be transferring your/your child’s healthcare records. It is highly likely the clinicians currently providing your/your child’s care will continue to do so.

We have provided some further information which you may find helpful. This also includes contact details for you to get in touch with us if you have any further questions or concerns before March 31 2020.
