Our Community Learning Disabilities team is made up of clinical professionals who specialise in working with adults who have a learning disability. We work with people to support their health needs in a variety of community settings, including at home or college, during our day service, in their place of work or Doctor’s surgeries. We also have hospital Liaison Nurses who are based in Southmead Hospital in Bristol.
We work with people to help them to understand and make choices about their health. Our teams are made up of a variety of clinical professionals who specialise in working with adults who have a learning disability, both directly with our service users and indirectly, offering support through family or support workers. We assist in setting goals and making plans to meet their needs.
Based in Bristol, South Gloucestershire and North Somerset, we provide community support across each of these localities.
Based at Southmead Hospital, Weston General Hospital and Bristol Royal Infirmary, the primary aim of our Learning Disability and Autism Liaison Service is to support individuals who have learning disabilities and / or autistic people to access acute hospital provision and specialist healthcare when needed. This is achieved through collaborative working with our service users, carers, community colleagues and hospital staff to provide reasonable adjustments enabling access to hospital facilities.
Our key goals ensure individuals receive the safe harm-free care they need by providing reasonable adjustments and timely intervention to facilitate access to outpatient appointments, supporting inpatient admissions, ensuring safe discharge, minimising delayed discharge and reducing the risk of avoidable re-admission.
