Community health services provide care in or close to people’s homes to help them feel supported, avoid having to go to hospital or help them when they come out of hospital.
These services work closely with general practices. Examples include community nursing, physiotherapy and speech and language therapy, among many others. NHS Bristol, North Somerset and South Gloucestershire CCG is the organisation that plans and pays for health services.
Contracts for community services are coming to an end over the next few years so we are planning how to offer consistent community services across the whole region. Our focus is on having a service that meets people’s needs both now and in the future, putting patients and carers at the centre of care. We want to hear what the ideal community services look like to you. Give us your feedback by taking our quick survey. It will only take five minutes and feedback will influence what happens in future.
We are also running workshops to help us plan. We are inviting partner organisations, clinicians, people who use services and their families and others to tell us what you think. 1 November 2018, 9.30-12.30 at St Michael’s Church Centre, The Auditorium, The Old School Rooms, The Green, Stoke Gifford, Bristol, BS34 8PD – the focus will be on intermediate care
1 November 2018, 1.30-4.30 at St Werburghs Community Centre, Horley Road Bristol BS2 9TJ – the focus will be on specialist support and advice, particularly nursing
15 November 2018, 9.30-12.30 at The Vassall Centre, Gill Ave, Bristol BS16 2QQ – the focus will be on locality hubs – buildings where a range of community services could be offered.
We will start each session with an overview of the process we are using and the overall vision. We will then do group work to hear more about the specific workshop topic.
